Please note there are two clinic locations. Mondays and Fridays in Silverlake and Thursdays in Atwater.

Booking - The credit card requested for booking is held on file for the cancellation fee, as appointments must be cancelled/rescheduled at least 24 hrs in advance. You may use any credit card, FSA, HSA, venmo, or zelle for payment at your appointment. I do not keep cash on hand for change.

New patients will receive an email with health intake forms and additional details after booking your initial session. If you are returning, but this is your first time seeing me at this location, please make sure to sign the HIPAA and Consent Forms that will be emailed to you.
